Temporary Leave-Home Preparation Checklist
Prepare manageable essentials and agreed help on a quiet day, before any need to leave home temporarily.
Tick on paper as you discuss what applies. A tick records a conversation or check, not a guarantee. Leave irrelevant items blank. This is not a sequence to complete during an emergency.
Talk through what would work
- Possible place to stay discussed; access and care needs checked.
- Assistance agreed with the people who may need it and those offering help.
- Transport/access options discussed; no assumption that a car or driver is available.
- Contact arrangements and important numbers available on paper.
- Medicines/care and equipment arrangements checked with the relevant care team or provider.
- Location of essential information noted without copying sensitive document numbers.
- Glasses, mobility aids and essential everyday items considered.
- Suitable clothing, phone/charging, familiar food and water considered for the circumstances.
- Pets or assistance animals included; possible destination checked.
- Know where to find current official/local instructions; these override earlier plans.
Own additions or next conversation — no absence schedule
- Notes
Print before you need it

Complete the printed sheet by hand. There are no online fields, saving or submission. Keep a paper copy available before disruption; this website is not an offline app.
